Clubs Referral Form

Please read our eligibility and FAQ pages prior to making a referral. We receive a significant number of referrals daily, many of which do not match the required criteria stated on our eligibility page.

*Please be aware that it can take up to 10 working days for us to process a referral. If you or the family have not heard from us after 10 working days please get in touch! Once you press submit the page will go blank, this means the form has been submitted correctly.

Please do not submit the same referrals multiple times within the initial 10 working day period.

**A phone number and email address for the Guardian is mandatory, please provide both on the form.

If a referral is received which does not match our eligibility, the referral will be rejected.
